Key Job Responsibilities

Triage & Assessment
Undertake timely, strengths-based assessments of need, risk, and safeguarding concerns for adults and carers. Screen and prioritise incoming contacts and referrals.
Care Planning
Develop and implement person-centred care and support plans that promote independence, recovery, and wellbeing
Safeguarding
Carry out safeguarding enquiries and properly escalate risks according to statutory legislation and local procedures (e.g., the Social Services and Well-being (Wales) Act 2014)
Multi-Agency Working
Collaborate closely with health colleagues, housing services, community resources, and the voluntary sector to deliver joined-up support and prevent crises.
Early Intervention
Champion preventative approaches to reduce unnecessary escalations and help individuals retain control.
