About this role
As a Band 6 UCB/SPA Nurse, you will provide specialist clinical assessment and triage via telephone, ensuring patients receive timely and appropriate care within community settings. You will assess complex referrals, identify risks and "red flags", and determine the most suitable care pathway, including escalation to emergency services where required. The role requires excellent clinical judgement, communication skills, and the ability to work independently while remaining closely connected to a wider multidisciplinary team. You will manage referrals from a variety of healthcare professionals, support patients with complex physical and mental health needs, and provide reassurance and expert advice to patients, carers, and referrers. You will contribute to service development through clinical governance, audit, quality improvement initiatives, and the implementation of evidence-based practice. In addition, you will support the education and development of colleagues and students, act as a role model within the team, and assist with the day-to-day operation of the service when required. The post operates on a flexible rota covering seven days per week and contributing to a 24-hour service, requiring a commitment to working evenings, weekends, and bank holidays as needed. The successful candidate will be passionate about delivering excellent patient care, promoting independence, and working collaboratively with partners across health and social care to achieve the best outcomes for patients.
