Bank level 7 Single Point of Access Clinical Practitioner

Provide advanced clinical triage, assessment and decision-making for patients with acute, complex or undiagnosed needs. Prioritise referrals, manage clinical risk and respond appropriately to urgent or emergency situations. Initiate investigations, interpret results, prescribe medication and refer patients to appropriate services. Work collaboratively with patients, carers and multidisciplinary teams to develop, monitor and review individual care plans. Support admission avoidance and coordinate safe discharge or transfer of care. Provide senior clinical leadership, supervision, training and support to colleagues. Maintain accurate clinical records and comply with safeguarding, clinical governance, confidentiality and infection-control requirements. Contribute to service planning, audit, quality improvement and the implementation of clinical policies and pathways.
