Clinical and Service Lead

Some of the key responsibilities but not limited to: To lead the CHC Team in the assessment and review of cases for eligibility and Funded Nursing Care where required, to ensure these are completed in a robust and timely way. This includes timely reviews of Fast Track patients. To deliver appropriate NHS Continuing Healthcare Training across Health and Social care ensuring that eligibility criteria are consistently applied. To review and monitor packages of care with appropriate multidisciplinary involvement ensuring placements are both appropriate and cost effective, seeking appropriate clinical advice where necessary. To address and respond to all complaints received arising from the decisions of the eligibility panel for NHS Continuing HealthCare Assessments To ensure that the Team Members provide the appropriate and relevant Support as identified and agreed with the Home staff, the resident and their relatives. To accept and co-ordinate referrals for Continuing Care /NHS Funded Nursing Care assessments, ensuring that the eligibility and funding criteria are identified accurately and best practice guidelines are adhered to.
