About this role
As a Mental Health Practitioner, you will play a key role in delivering specialist community mental health support to adults experiencing severe and enduring mental health difficulties. You will undertake holistic assessments, formulate care plans, coordinate support packages, and provide therapeutic interventions tailored to individual needs. Using your professional expertise, you will establish and maintain therapeutic relationships with service users, helping them navigate periods of crisis, recovery, and ongoing support. You will work collaboratively with individuals, carers, and professionals to identify strengths, build resilience, and promote recovery-focused outcomes. You will be responsible for assessing and managing clinical risk, safeguarding vulnerable adults and children, and communicating complex and sensitive information effectively to individuals, families, and professional partners. The role requires sound clinical judgement, excellent communication skills, and the ability to work autonomously while remaining an integral part of a multidisciplinary team. The successful candidate will have experience working within community mental health services and a strong understanding of health and social care systems. You will contribute to service development, audit activities, clinical governance, and continuous improvement initiatives, ensuring high standards of care are maintained at all times. You will also be expected to provide supervision and mentorship to Band 5 practitioners, unregistered staff, and students, supporting the development of future professionals and fostering a positive team culture.
