Advanced Nurse Practitioner/Advanced Clinical Practitioner

Key Responsibilities Independently assess, diagnose and manage patients presenting with undifferentiated and complex health needs. Provide evidence-based treatment, advice and health education. Prescribe medication safely and appropriately within professional competence. Manage acute illness, minor ailments and long-term conditions. Request, interpret and act upon investigations and diagnostic tests. Work collaboratively with GPs and the wider multidisciplinary team to provide integrated care. Maintain accurate and timely clinical records. Contribute to quality improvement initiatives, clinical audits and service development. Support the practice in achieving high standards of patient care and clinical governance.
