Care Coordinator/Navigator - Woodgate Valley Health Centre

Key Responsibilities Will Include

Greeting patients and visitors in a professional and compassionate manner, in person and over the telephone Book, reschedule and cancel patient appointments using the clinical system Handle incoming phone calls, triage queries and signpost patients to the most appropriate clinician or healthcare service Process prescription requests and liaise with GPs and Pharmacists where necessary Register new patients and maintaining accurate patient records using clinical systems Manage patient queries regarding test results, referrals and general practice information Handling confidential information in line with GDPR, NHS Guidelines and practice policies Supporting clinicians and the wider practice team with administrative duties Assist with referrals to secondary care and community services Process incoming and outgoing correspondence Scan, file and code clinical documents into patient records Please see the attached Job description for the full details of the role.
