Health & Social Care Assessor

The service will be delivered in accordance with the principles of Recovery; empowering individuals to take control of their lives and putting them at the centre of everything we do, recognising the rights of all individuals to gain access to valued roles, relationships, activities and facilities within their local community. The post holder will facilitate the patients safe transfer from hospital back into the community and to liaise between the hospital and other statutory and non-statutory agencies as required. The post holder is expected to represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of Health & Social Care understanding and professional judgement. The post holder is responsible for working independently, exercising assessment and evaluation skills to full ability in order to adjust care assessments as professionally judged to be necessary, and instigate changes in care that are evidence based and in the patients best interests.
