Community Nursing Teams provide personalised nursing care to housebound patients aged 18+ with complex and long-term conditions, helping them remain safe, independent and well in their chosen environment. Working as part of an integrated multidisciplinary team aligned to a Primary Care Network (PCN), we focus on preventing avoidable hospital admissions.

As aClinical Lead, you will be an experienced Community Nurse who provides clinical leadership, sound professional judgement and expert decision-making. You will act as a role model for the profession, representing Community Nursing within your PCN while leading and developing a team committed to delivering outstanding care.

Our teams provide a wide range of interventions including palliative and end of life care, medication administration including IV therapy, wound management, bladder and bowel care, and supporting patients to manage long-term conditions.

This role is primarily Monday to Friday; however, flexibility is required to support a seven-day service operating between 08:00 and 18:00, including weekends and bank holidays.

As Clinical Lead, you will be accountable for the quality, safety and performance of your team. You will provide day-to-day leadership, supervision and professional support, ensuring excellent standards of care and compliance with professional, organisational and regulatory requirements.

Key responsibilities include leading caseload management, supporting staff development through supervision, appraisal and training, undertaking recruitment and induction activities, and promoting a culture of continuous improvement. You will contribute to clinical governance, audits, quality improvement initiatives, service performance targets and risk management processes.

Working collaboratively with patients, carers, GPs, care homes and wider health and social care partners, you will build strong professional relationships and represent Community Nursing within multidisciplinary forums. You will support evidence-based practice, participate in audit and research activity, and help shape service development to ensure the best possible outcomes for patients.

If you are a motivated and experienced Community Nurse looking for an opportunity to lead, inspire and develop others while making a real difference in your community, we would love to hear from you.
