About this role
Work as an integral part of the multidisciplinary team to coordinate and administer long-term condition reviews, childhood immunisation programmes and cervical screening recalls across the practice population. Run and monitor clinical system reports to identify patients due, overdue, or approaching their review, vaccination, or screening dates. Ensure eligible patients are invited at the appropriate intervals for LTC reviews, childhood vaccinations, and cervical smears in line with national guidance, QOF requirements, and practice protocols. Proactively contact patients, parents, or carers by telephone, text message, letter, or other approved communication methods to arrange appointments and maximise uptake.Support the monitoring of childhood vaccination schedules, ensuring children are called in at the correct ages and intervals to maintain immunisation targets. Help identify children who have missed vaccinations and support timely follow-up to improve uptake and reduce missed opportunities. Monitor and manage cervical smear recalls, ensuring eligible patients are invited promptly and non-responders are followed up appropriately. Maintain accurate tracking systems to monitor invitations sent, bookings made, non-attendance, completed reviews, vaccinations, and smear appointments. Review and investigate missing, incomplete, or outstanding patient reports to ensure no eligible patients are omitted from recall programmes. Identify and escalate gaps, trends, and missed clinical opportunities, highlighting patients who may have been missed for LTC reviews, childhood immunisations, or cervical screening. Work closely with GPs, nurses, HCAs, practice management, and administrative teams to support safe and effective recall systems.
