We are looking for an EIS Senior Practitioner to join our Affinity Team, supporting the delivery of Hertfordshire's Early Intervention & Prevention Service (EIS) and guiding the EIS Practitioners who deliver it. EIS provides targeted, time-limited support for children and young people at risk of escalation, including those at risk of exclusion or persistent absence, with emerging emotional wellbeing needs, neurodiverse needs affecting daily functioning, or family and environmental stress. The service is formulation-led, trauma-informed and system-integrated, working with families, schools and the wider professional network to build a shared understanding of need and strengthen the people and environments around the child. In this role you will embed the Framework for Integrated Care and the Rule of 167, coordinate multi-agency working, facilitate formulation-informed conversations, manage a direct caseload and provide practice support and guidance to the EIS Practitioner, helping to ensure consistent and effective delivery of the service model. This is a full time, fixed term position for 2 years based at our Harlow Family Hub, with Hertfordshire-wide delivery across education, community and family settings, in response to the needs of children, young people and the service. Main Responsibilities Provide senior practitioner support within EIS Hertfordshire, contributing to the safe, consistent and high-quality delivery of the commissioned model. Facilitate formulation-led practice, bringing together the perspectives of the child or young person, family and relevant professionals to support a shared understanding. Contribute to referral, triage, prioritisation and suitability discussions in line with agreed pathways and the service prioritisation framework, seeking guidance from the Affinity Team Lead where required. Provide practice support and case discussion opportunities for the EIS Practitioner, helping to ensure demand, complexity, risk and capacity are appropriately managed. Hold and manage an allocated caseload, undertaking assessment, formulation, intervention, review and closure, keeping support targeted, time-limited and focused on agreed outcomes. Provide senior input in more complex situations and escalate concerns, risks or service decisions to the Affinity Team Lead as appropriate. Coordinate and contribute to multi-agency formulation discussions, ensuring relevant partners are involved and plans are aligned around the child or young person. Build and maintain effective working relationships across Hertfordshire with education settings, CAMHS and Mental Health Support Teams, social care, early help, youth justice and voluntary and community sector partners. Act as a connector across the professional network, reducing duplication, addressing gaps and supporting clear communication and shared planning. Work alongside schools, parents/carers and trusted adults to strengthen their understanding of, and response to, the child or young person. Keep the voice and lived experience of children, young people and families central to formulation, goal setting, planning and review. Model trauma-informed, relational and psychologically informed practice, and support reflective practice and shared learning within the team. Support the development and delivery of consultation, training and practical resources for schools, families and partner agencies. Maintain accurate, timely records and contribute to service monitoring, outcome tracking and continuous improvement. For a full list of job responsibilities please see the attached job description. Ideal Candidate Significant, demonstrable experience of working with children, young people and families within a relevant service. Experience of implementing the Framework for Integrated Care and/or Rule of 167 approaches, and the ability to embed them in day-to-day practice. Ability to help children, families and professionals develop a shared understanding of strengths, challenges, relationships and environmental influences. Ability to identify and strengthen naturally occurring support networks around children and young people. Experience of facilitating network meetings, Team Around the Family meetings or multi-agency planning, and the ability to build consensus across professional perspectives. Experience of working in schools or education settings. Experience of delivering 1-1 interventions to children, young people, parents or carers, and of delivering training, workshops or consultation. Ability to engage children and young people who may be reluctant to access traditional services, including in group activities. Understanding of neurodiversity-affirming practice and of using goal-based outcomes. Excellent communication skills with children, young people and adults, with the ability to influence both strategically and corporately. Clear understanding of children's and adults' safeguarding concerns and the implications for practice. Sound decision making and the ability to exercise effective judgement. A valid driving licence and access to a vehicle for travel to schools and partner organisations. About the Company We change lives by transforming health and care.
