Community Senior Hospice at Home Nurse (Night Team)

Carry out holistic assessment of patients and those close to them, and develop, deliver and evaluate individualised care plans. Deliver fundamental and complex specialist interventions in the home, including symptom management within the RGP medicines framework, syringe driver care and, once competent, verification of expected death. Respond to

SOS and unplanned calls

assess, act within your competence, and escalate to the CNS, out of hrs advice line, or 111 when the situation needs decisions beyond your scope. Deliver hands-on care when it is right for the patient, including on crisis response and unplanned care visits. Communicate sensitive information with skill and care, supporting families before and after bereavement. Keep accurate, contemporaneous records and hand over clearly across shifts and services.
