Key responsibilities will include
Undertaking medication reviews, including Structured Medication Reviews (SMRs), to optimise treatment and improve patient outcomes. Developing effective working relationships with community and hospital pharmacy colleagues and other healthcare professionals to enhance patient care. Supporting the repeat prescribing and reauthorisation process by reviewing repeat prescription requests and identifying medicines requiring review. Responding to medicines-related queries from patients and healthcare professionals. Improving prescribing quality and safety through clinical searches, recalls, audits and Plan-Do-Study-Act (PDSA) cycles. Supporting the management of practice formularies to promote safe, effective and cost-conscious prescribing. Implementing medicines safety alerts and drug withdrawals, including those issued by the Medicines and Healthcare products Regulatory Agency (MHRA). Working with practice clinicians and patients to implement NICE guidance and other evidence-based recommendations. Undertaking clinics where medicines optimisation is a key intervention, including SMRs and appropriate chronic disease monitoring clinics. Reviewing relevant pathology results for patients taking prescribed medicines and supporting medication reauthorisation where clinically appropriate. Contributing to multimorbidity reviews and multidisciplinary team discussions. Providing medicines information, advice and training to practice healthcare professionals, administrative staff and patients. Acting as a source of medicines expertise for the wider practice team, advising on dosing, side effects, adverse reactions, medicine availability and appropriate alternatives. The postholder will also contribute to practice priorities, clinical quality improvement and the ongoing development of pharmacy services, working collaboratively with colleagues to maintain high standards of patient care and medicines safety.
