Responsibilities

Assist patients with daily activities such as personal hygiene, dressing, and mobility

Support the implementation of individualised care plans tailored to each patient's needs

Monitor and record patients' health status, reporting any concerns to senior staff

Provide companionship and emotional support to patients, fostering a positive environment

Assist with medication administration following prescribed guidelines (training provided)

Maintain accurate documentation of care provided using IT systems

Support with light domestic tasks to ensure a clean and safe environment

Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery

Drive between different care locations as required, ensuring timely patient support
