We are looking for caring and reliable Community Hub Coordinator to join our team at Fern House, Bingley

Working hours are -

Wednesday 8

30am to 4:30pm

This role is initially for one day per week with the view to increase that to 2 days by Christmas.

Key Duties

Support the day-to-day running of the Community Hub, ensuring a high-quality, person-centred service.

Provide care and support that promotes residents’ independence, dignity and overall wellbeing.

Assist with assessing individual needs and reviewing person-centred care plans.

Organise and support a range of activities, events and community initiatives to meet residents’ needs and interests.

Build positive relationships with residents, families, colleagues, volunteers and external professionals.

Encourage residents to participate in local community activities and maintain social connections.

Support the Community Hub Coordinator in monitoring staff performance and maintaining service standards.

Ensure compliance with relevant legislation, regulations, safeguarding requirements and organisational policies.

Promote continuous improvement and help deliver a safe, effective and high-quality service.

Attend required meetings and training, maintaining ongoing professional development and awareness of best practice.
