Advanced Clinical Practitioner (ACP)

Main Job Responsibilities (Refer JD for detailed information) Undertake advanced assessment, diagnosis, care planning and treatment for people with complex physical and mental health needs. Prescribe, monitor and review medicines and other interventions within professional and governance requirements. Identify and manage clinical risks, respond to deterioration and escalate concerns appropriately. Work with service users, carers, multidisciplinary teams and partner agencies to deliver coordinated, person-centred care. Provide clinical leadership, expert advice, supervision, education and support to staff and learners. Lead and contribute to service development, quality improvement, audit, research and evidence-based practice. Participate in incident reporting and PSIRF processes, applying learning to improve quality and safety. Maintain professional standards, governance requirements and continuing professional development across the four pillars of advanced practice.
