About this role
Deliver advanced clinical assessment, diagnosis, treatment and management of patients presenting with frailty, multiple long-term conditions, acute illness and urgent care needs. Independently manage same-day urgent care clinics, assessing patients with undifferentiated presentations and determining appropriate treatment, referral or escalation. Undertake home visits, care home reviews and urgent community assessments, ensuring patients receive care in the most appropriate setting. Provide rapid assessment and intervention for patients at risk of deterioration, supporting admission avoidance and reducing unnecessary hospital attendance. Manage acute and complex presentations, providing treatment, follow-up and ongoing management where clinically appropriate. Conduct telephone, digital and face-to-face consultations, including clinical triage of patients presenting with urgent and unscheduled care needs. Refer patients appropriately to internal and external services, ensuring timely access to specialist assessment, investigation and treatment where required. Support and educate other members of the multidisciplinary team, acting as a resource for advanced clinical practice, frailty management and urgent care. Participate in multidisciplinary team meetings, case reviews and care planning discussions to support coordinated patient care. Perform comprehensive frailty assessments and develop personalised care and support plans for patients with complex health and social care needs. Deliver proactive case management for housebound patients, care home residents and those identified as living with moderate to severe frailty. Request, interpret and act upon clinical investigations and prescribe safely and effectively within scope of practice. Coordinate care across primary care, community services, secondary care, social care and voluntary sector organisations to ensure seamless patient journeys. Work collaboratively with GPs and the multidisciplinary team to improve access, manage clinical demand and support delivery of same-day care services. Provide specialist clinical leadership, advice and support to colleagues, patients and carers. Contribute to safeguarding, clinical governance, audit, quality improvement, service development and implementation of evidence-based practice.
