About this role
To assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
Chronic Disease Management to include
Diabetes, Asthma, COPD, Chronic Heart Disease, Hypertension, to develop and implement care plans for patients with Long term conditions, promoting self-care. Cytology, Childhood immunisations, NHS Travel vaccinations, phlebotomy, womens health, injections To keep accurate clinical detailed health records on Emis. Undertake mandatory and statutory training requirements.
