About this role

Deliver holistic occupational therapy assessments for individuals with complex physical, mental health and social care needs, including functional, environmental and risk assessments to promote independence, wellbeing and safety within the home and community. Develop comprehensive personalised care and support plans in partnership with patients, families and carers, identifying strengths, needs, risks and goals that matter most to the individual. Provide specialist occupational therapy interventions that promote reablement, self-management, hospital avoidance and improved quality of life. Assess for, prescribe and facilitate appropriate low-level equipment and minor adaptations to maximise independence and reduce risk. Work autonomously to manage a complex caseload, using advanced clinical reasoning and professional judgement to assess risk and determine appropriate interventions. Provide specialist occupational therapy advice, guidance and support to colleagues within the multidisciplinary team, contributing to effective clinical decision-making and person-centred care. Support the induction, supervision, mentoring and development of staff, students and peers as required. Work collaboratively with primary care, community health services, social care, housing and voluntary sector partners to deliver coordinated care. Advocate for patients and support access to services, resources and community-based support that address wider determinants of health. Contribute to service development, quality improvement, outcome measurement and the reduction of health inequalities, while maintaining professional standards, clinical governance requirements, safeguarding responsibilities and accurate clinical documentation.
