About this role
Provide a counselling service for patients and their families known to the Hospice both pre and post bereavement and also for bereaved clients outside of the hospice Support the Service Lead with operational service efficiency, data management information, colleague support and appropriate triaging and allocation. Identify the nature, severity and complexity of patients and families requirements through assessment to formulate and implement a plan for their care Maintain clinical records in line with the Hospices policies and procedures
