About this role
To work within the Childrens Community Nursing Service, delivering high quality, evidence-based nursing care for children and young people with life limiting and life threatening conditions and supporting children and young people with short- and long-term conditions. To identify the health needs of children to be nursed in the community and work in liaison with multi-agency partners to promote collaboration and effective communication.
