Experienced Practice Nurse - The Ridge PCN

Support public health and screening programmes, including immunisations, vaccinations, where appropriate. Completing Respect forms. Deliver expert nursing assessments and manage complex care needs of care home residents within Ridge PCN in partnership with GP. Support patient and public involvement to enhance patient care and service delivery. Create a trusting partnership with patients and/or relatives/carers to communicate and explain complex medical issues Mentor, support, and supervise new-to-general-practice nurses, student nurses, and other healthcare professionals (e.g., nursing associates, HCSWs) across the PCN by agreement and when appropriate.
