About this role

Why this role would be great for you

We are seeking a compassionate, motivated, and experienced Senior Housing Support Practitioner to join our step-down services in the community. This is an exciting opportunity for someone who is passionate about supporting people to achieve greater independence and is confident using their initiative when working independently. Our mental health services provide residents with a move-on opportunity, supporting them as they transition towards independent living within the community.

The successful applicant will manage a large caseload and provide person-centred support focused on resettlement, community engagement, wellbeing, and tenancy sustainment. You will work closely with residents to help them develop the skills, confidence, and independence required to successfully move on from the service.

The role will also involve responsibility for the day-to-day management of the building, including health and safety, safeguarding, and maintaining a safe and welcoming environment for residents.

As the role will involve lone working, the successful applicant will need to be confident working independently, managing their own workload, making appropriate decisions, and responding effectively to situations as they arise. A visible presence at Nightingale House (L20) will also be an important part of the role.

What your week may look like

The Senior Housing Support Practitioner will be skilled in supporting residents with resettlement, managing expectations, whilst building positive therapeutic relationships.

The post holder will also ensure the health and safety of the buildings we manage. This includes weekly fire checks, reporting any maintenance concerns, amongst other tasks.

Although the support worker will work in the community, they will be part of the Nightingale Leadership Team. This will include attending team meetings, alongside reflective practice at the service, and may be expected to facilitate supervision sessions for support practitioners in line with service needs.

Some of the expectations of the role include

Managing time effectively to support our outreach residents.

Ability to use initiative and think outside the box

Ensuring each person will receive personalised support considering their identified risks and personal needs

Engaging with professionals and other providers to provide a safe space to complete support sessions.

Providing a safe, welcoming environment with excellent standards and an ethos of service delivery that encourages all service users to engage with support on offer

To respond to emergency referrals and/or situations.

It is recognised that the association is operating in a dynamic environment and as such this job description may be varied in consultation with the post holder to reflect evolving needs of the business and changing nature of the job.
