Care Coordinator

We are a friendly and dynamic care provider seeking a proactive, skilled and dedicated full-time Care Coordinator.

The ideal candidate will be highly organised, efficient, and confident in managing the varied tasks involved in delivering exceptional care. A valid UK driver’s licence and access to your own transport are essential for this role.

Duties

* Advise and direct care staff in their role to ensure safety and efficiency

* Meet and arrange care for new clients, making assessments of care needs and liaising with healthcare professionals

* Conduct reviews of clients and do staff supervisions

* Take phone calls, arrange staff rotas and training sessions

* Maintain industry and CQC standards of care provision

* Provide hands-on care in the community during emergency situations where immediate care is needed

* Help develop our services throughout Oxfordshire through networking and service promotion

* Hold out-of-hours phone on a three week rotational basis.
