Clinical Pharmacist

As a Clinical Pharmacist, you will provide expert pharmaceutical care and demonstrate safe clinical decision-making within general practice. Working within your professional competence and scope of practice, you will: Undertake structured clinical medication reviews for patients with complex and long-term conditions. Provide medicines optimisation support to improve patient outcomes and reduce medicines-related harm. Manage repeat prescribing processes and support safe prescribing systems. Support the management of long-term conditions where medicines optimisation is required. Implement medicines safety alerts, national guidance, NICE recommendations and local formulary changes. Contribute to quality improvement initiatives and clinical audits. Provide education and medicines advice to clinicians, practice staff and patients. Contribute to public health initiatives, including vaccination campaigns and health promotion activities. Work collaboratively with community pharmacies, secondary care colleagues and wider healthcare teams to ensure continuity of patient care.
