Neurology Specialist Nurse

Actively facilitates and participates in improving and enhancing the delivery of the care given to individuals with a long-term neurological condition, at a local, regional and national level. To triage nurse specialist referrals for individuals with a long-term neurological condition, who have a GP within West Norfolk. To provide assessment, monitoring, support and education for people with the identified neurological conditions using recognised assessment tools, evaluate findings and develop a goal plan. To monitor drug therapies and recommend changes as appropriate in line with professional guidelines (Nursing and Midwifery Council) and local and national guidelines.
