The following outlines some of the key responsibilities of the successful candidate. We are happy to discuss arrangements as to part time, full time and flexible working. Our core service hours are Monday - Friday 8am - 4pm. assisting with new patient registrations and supporting patients to access the service. handling requests for information from patients and external organisations with support from service leads. working with hospitals and outpatient care providers to enable our patients to access healthcare. managing post received for patients using the practice as their care of address. supporting with data collation and patient follow up. participating in multidisciplinary team meetings. responding to enquiries from patients and external organisations, providing support and signposting to other agencies where appropriate. supporting the outreach team to engage patients to attend appointments at the practice and elsewhere. ability and willingness to support our nurse led outreach by driving our clinical van is beneficial but not essential. due to the nature of our service an understanding of safeguarding responsibilities is essential, training will be provided to the successful candidate. contributing to ongoing development of the homeless healthcare service working alongside statutory, third sector and voluntary services.
