Responsibilities

Conduct comprehensive assessments of adults in need of social care support.

Develop and implement individualized care and support plans.

Coordinate services with healthcare professionals, community organizations, and families.

Provide guidance and advocacy to empower clients in making informed decisions.

Monitor and review the progress of clients' care plans regularly.

Maintain accurate and timely records in accordance with data protection standards.

Stay up-to-date with relevant legislation, policies, and best practices in adult social care.

Work collaboratively within a multidisciplinary team to deliver holistic support.
