About this role
The remit includes (but is not limited to) the following: Hold operational leadership for a defined portfolio of complex care, ensuring effective commissioning, review and assurance of individual packages of care. Hold expert understanding of the legal and policy requirements related to SEND, Learning Disability, neurodiversity and mental health packages of care depending on portfolio role. Act as the senior point of coordination for complex cases within the portfolio, ensuring timely decision making and resolution of quality, placement or cost issues. Manage, report and monitor CETR, CTR and other meetings and reviews within the timescales required. Some face-to-face meetings may require travelling out of the borough Be responsible for the Dynamic Support Register arrangements as appropriate Lead multi-agency decision making for complex cases including Local Area Emergency protocol (LAEP) meetings Take a population health management approach to understand current and future need Contribute to the development and implementation of commissioning and market management plans by providing operational insight form casework and provider partnerships. Manage relationships with Local Authorities, set up assurance meetings as required Oversee and maintain patient registers Lead on a KLOE tracker and report Maintain a tracker including a tracker for discharge plans which includes S117 applications, capacity assessments, risk assessments and barriers to discharge. Oversee the completion of Root Cause Analyses to identify where improvements to care can be made Maintain a view of spend against budget and developing plans to support better value for money Lead on improvement and CIP plans as required.
