About this role
The post holder will play a key role in supporting patients to improve their health, wellbeing and independence through a personalised, person-centred approach. Working closely with GP practices, community services, voluntary organisations and the wider integrated Neighbourhood Team. The role will support patients to identify goals, develop self-management skills access community resources and address the wider determinants of health. The role will focus on supporting frail, vulnerable and at risk patients through both proactive and reactive interventions, helping to reduce health inequalities, prevent crisis situations and avoid unnecessary hospital admissions. The post holder will work within agreed protocols and their scope of practice, escalating concerns to registered clinicians where appropriate. This is a non-clinical role that acts as a bridge between primary care services and the realities patients face in their daily lives, ensuring support is based on what matters most in individuals.
