Job Description

Personal assistant to work with Client to help grow and encourage her to have a more independent and

happy lifestyle.

Roles and Responsibilities

Client Support Needs

Client would benefit from regular support and encouragement across a range of daily living activities to help maintain her independence, wellbeing, and community involvement.

Daily Living and Household Support –

Support may include

Encouragement and assistance with completing weekly laundry, including washing and changing clothing, bedding, and towels.

Support with weekly shopping for groceries, personal hygiene products, and clothing.

Assistance with meal planning, food preparation, and cooking to promote healthy eating and maintain independent living skills.

Weekly support with household cleaning and domestic tasks, including hoovering, washing pots, tidying, and maintaining a safe and clean home environment.

Encouragement to participate in household chores independently where possible, with prompts and guidance as required.

Health and Medical Appointments

Client requires support to attend and manage healthcare appointments, including

A blood test appointment every four weeks, held on a

Monday at 11

00am.

Six-monthly outpatient appointments.

GP appointments as required.

Hospital appointments as required.

Support may include appointment reminders, transport, accompanying Client to appointments, and providing reassurance if needed.

Community Access and Social Inclusion

Client enjoys accessing her local community and would benefit from support to maintain social engagement and meaningful activities, including:

Attending an art class on most

Tuesdays from 1

00pm to 3:00pm.

Going for walks in local parks.

Visiting shopping centres and accessing community facilities.

Participating in activities of interest to promote wellbeing, confidence, and independence.

Family Contact

Maintaining family relationships is important to Client. She will require support to visit her father once per week, including assistance with planning, transport, and accompaniment where necessary.

About Me

Client is a friendly, caring, and personable individual who enjoys spending time with others and engaging in activities within her local community. She has a diagnosis of Paranoid Schizophrenia and, at times, may experience auditory hallucinations, where she hears voices. Client has developed a good understanding of her condition and is generally able to distinguish between experiences related to her illness and reality.

Client manages her symptoms well with the support of her prescribed medication and demonstrates good insight into her mental health needs.

Client has a warm and outgoing personality and enjoys social interaction. Her interests include watching television, listening to music, shopping, attending art classes, and spending time out in the community. She also enjoys walks in local parks and visiting places of interest. Maintaining contact with family is important to Client, and she values opportunities to visit her father regularly.

With the right encouragement and support, Client is motivated to maintain and further develop her independence, confidence, and daily living skills. She responds well to positive reinforcement and benefits from support that promotes choice, inclusion, and participation in meaningful activities.
