consistent presence for our residents.

This is a rewarding role where you will work closely with residents, their families and partner agencies to provide

housing-related support that promotes independence, wellbeing and tenancy sustainment. You'll play a key role in helping residents

remain safe and connected within their community, whilst supporting them to access services, maintain their quality of life, and

achieve positive outcomes.

As part of a dedicated Extra Care team, you will provide a visible and responsive service, support residents through changes in

circumstances, and create opportunities for health, wellbeing and social engagement.

What you'll be doing

* Providing housing-related support that helps residents live independently and sustain their tenancies

* Carrying out property viewings and promoting the benefits of Extra Care living to prospective tenants

* Completing needs and risk assessments and developing personalised support plans tailored to individual needs

* Supporting residents to settle into their homes, understand their tenancy responsibilities, and access relevant services and

support

* Monitoring residents' wellbeing and reviewing support plans to ensure they remain effective and responsive to changing needs.

* Working closely with families, care providers and partner agencies to coordinate support and achieve positive outcomes.

* Arranging adaptations and assistive technology to help residents maintain their independence and quality of life.

* Collaborating with Allocations and Voids teams to support new tenancy sign-ups and minimise void rent loss.

* Developing and delivering health, wellbeing and social activities that encourage participation, independence and community

involvement.

* Building connections between residents and the wider community to reduce social isolation and promote active lifestyles.

* Maintaining accurate records, contributing to audits and supporting the effective performance of the scheme.

* Promoting safeguarding, equality, diversity and inclusion, ensuring services are safe, responsive and person-centred.

What we're looking for

* Experience working in housing, social care, health, support services or a similar role supporting older or vulnerable people

* Experience carrying out assessments, support planning and providing person-centred support

* Knowledge of safeguarding and a commitment to promoting independence, wellbeing and positive outcomes

* Strong communication skills and the ability to build positive relationships with residents, colleagues and partner agencies.

* Confidence managing challenging situations and adapting to changing resident needs.

* Experience helping people access support services, welfare advice or budgeting support

* Good IT skills and the ability to maintain accurate records

Interview Process

Interviews will take place at The Maples, Kirkby on Friday 21st August 2026 and will consist of competency-based questions, giving

you the opportunity to demonstrate your skills, experience and suitability for the role.
