About this role

Essex Partnership University NHS Foundation Trust (EPUT) has an exciting opportunity to join our Care Coordination Centre (CCC) and Urgent Community Response (UCR) Team.

This full-time, fixed-term role plays a vital part in supporting the delivery of safe, responsive, and high-quality care for patients across the community. The postholder will work as part of a dynamic, fast‑paced service that ensures individuals receive the right care, at the right time, in the right place.

The Care Coordination Centre and UCR team work closely with multidisciplinary teams, partner organisations, and healthcare professionals to support hospital avoidance, timely discharge, and rapid community-based interventions.

The postholder will be responsible for

  • Acting as a central point of contact for referrals into the Care Coordination Centre and Urgent Community Response service
  • Receiving, processing, and triaging referrals in line with service pathways and priorities
  • Coordinating care and communicating effectively with clinicians, patients, carers, and partner agencies
  • Maintaining accurate and timely records using Trust clinical systems
  • Supporting multidisciplinary team working to ensure seamless patient journeys
  • Escalating concerns appropriately and contributing to service flow and patient safety
  • Providing excellent customer service in a fast‑paced and pressured environment
  • Supporting service audits, data collection, and performance monitoring where required
