About this role

Essex Partnership University NHS Foundation Trust (EPUT) has an exciting opportunity to join our Care Coordination Centre as flow tracker admin suport to the Transfer of Care Team . The role is a fixed term contract for a period of 8 months depending on start date.

The Flow Coordinator plays a key role in supporting patient flow and care coordination across services by ensuring accurate, real‑time management of patient information. The post holder is responsible for maintaining flow trackers, registering patients onto Electronic Patient Record (EPR) systems, and supporting Referral Coordinators with patient registrations and pathway updates. The role ensures that all patient care information is current, complete, and accessible to support safe, timely, and effective care delivery.

Patient Flow & Data Management

  • Maintain and update all patient flow trackers, ensuring information is live, accurate, and up to date at all times
  • Monitor patient pathways and proactively update changes in patient status to support effective patient flow
  • Identify and escalate discrepancies or delays in patient information that may impact care delivery

EPR & Patient Registration

  • Accurately register patients onto the Electronic Patient Record (EPR) system in line with Trust policies and data quality standards
  • Support Referral Coordinators with registering patients onto EPR, ensuring timely and correct data entry
  • Ensure all demographic, referral, and pathway information is complete and recorded correctly

Care Coordination Support

  • Update all relevant patient care information, including referral details, care plans, pathway milestones, and service involvement
  • Liaise with multidisciplinary teams to ensure patient information reflects current care needs and service activity
  • Support operational teams by providing accurate flow data to aid planning and decision-making

Communication & Team Working

  • Communicate effectively with clinical and non-clinical staff to ensure accurate flow and referral information
  • Respond to queries regarding patient registration and pathway status in a professional and timely manner
  • Work flexibly within the team to support service needs and priorities
