Clinical Responsibilities The post-holder will be expected to: Undertake a variety of consultations including face-to-face surgeries, telephone consultations, online/eConsults, and home visits where clinically indicated (home visits are infrequent). Provide assessment, diagnosis, treatment, referral and follow-up for patients of all ages presenting with undifferentiated and undiagnosed problems. Provide ongoing management of patients with acute and chronic conditions in line with NICE, local and practice guidelines. Issue and manage acute and repeat prescriptions in line with safe prescribing principles, the practice formulary and relevant local prescribing guidance. Review and act on pathology results, hospital correspondence, discharge summaries and other clinical documents in a timely manner. (Admin burden is light for this role) Make appropriate use of investigations and referrals, ensuring effective use of NHS resources. Contribute to the duty doctor rota and same-day urgent care provision on an equitable basis. Participate in chronic disease management and contribute to the achievement of QOF, IIF, enhanced services and other agreed clinical targets. Recognise and manage safeguarding concerns relating to children and adults at risk, in accordance with local and national safeguarding procedures. Maintain accurate, contemporaneous, legible and confidential patient records on EMIS Web.
