Care Worker

About this role

Must hold a valid UK driving licence

Assist service users and clients with: -

 Actively participate in the delivery of the care and support as identified in the

Individual Support Plan

 Assists in the assessment and re-assessment procedure for new and existing service users and clients

 Personal hygiene requirements

 Bathing in bed, bathroom or other

 Care of pressure areas

 Looking after skin, teeth, mouth, hair and nails

 Positioning and moving of service users and clients who are immobile

 Medication – where this is covered (together with the type of assistance) in the

Individual Support Plan

 Assistance with toileting and the cleaning and emptying of commodes

 Assisting with physical activity as needed, e.g. dressing, undressing, walking sitting, passive exercises etc.

 Position individuals correctly in bed, chair referring at all times to the risk

assessment for guidance with Multi-Care Community Service Ltd moving &

Positioning policy

 Assist individuals to eat and drink, as needed, being aware of nutritional needs and cultural requirement: reporting any unexpected inability or desire to eat or drink

 Ensure that any significant changes to an individual health and emotional condition are communicated to a line manager without delay

 To support the Community Nurse (or other competent person) in delivering care and support to a dying individual, offering support to relatives and friends

 Food & drink preparation

 Laundry, general cleaning and housekeeping duties

 Participating with an individual in social, leisure and recreational activities

 Talking and acting as a companion

 Accompanying the individual on trips out, if the requirement is agree following a

care and support assessment

 Shopping

 Assisting the individual to manage their personal affairs

 Collecting pensions or other benefits, if agreed on the Individual Support Plan

 Helping the individual maintain contact with family and friends

 Maintain accurate records in respect of care and support given, tasks undertaken, monies transacted etc.

 Participate in meetings and reviews where the workers’ direct knowledge of the

individuals’ care & support is an essential element in the process
