About this role
Purpose Manage the administrative workload arising from the practice's QOF, NEL LTC Proactive Care QOF and NC LES (including the Supplementary Care Home Service), ensuring targets are met and clinicians are freed from avoidable paperwork. Key duties Run and maintain recall systems for all LTC cohorts (diabetes, hypertension, CVD, CKD, COPD, asthma, heart failure) and care home residents. Book and coordinate Part A and Part B review appointments and invitations. Track completion of the diabetes eight care processes and chase outstanding items. Ensure accurate EMIS coding for QOF/PCQOF/LES activity (Part A 6AK, Part B 8CMD, IC Complex codes, care home SNOMED codes) on the day of activity.
Monitor time-sensitive care home tasks
48-hour post-discharge contacts, 111/crisis re-direction worklist, and SMR scheduling. Maintain accurate registers, records and the care home bed list. Support performance tracking against CEG/Edenbridge KPI dashboards. Prepare data for quarterly returns, the annual Q4 submission and reconciliation deadlines. Coordinate MDT meeting logistics and documentation. Act as administrative liaison between the practice, PCN and wider clinical team.
