About this role
Job Purpose
To look after the physical, emotional, cultural and social needs of the Service Users (SU) using a person-centred approach. Provide support to SU by facilitating, developing, monitoring and evaluating individual Support. Observe and promote SU’s rights to include choice, independence, dignity, privacy and fulfilment. Create and maintain good professional relationships with SU, their family and friends and other stakeholders. Actively support your colleagues and maintain a good professional relationship with them. Adhere to all regulatory and statutory obligations and LCC policies, procedures and guidelines. Be a positive ambassador of LCC and promote a personal and professional profile, ensuring you maintain the good reputation of LCC at all times.
Job Responsibilities
Support provision and Promotion of Quality Support
Documenting support and progress towards desired support outcome; identifying and correcting digression from the documented plan of support, reviewing progress with SU in conjunction with the principals of LCC
Reporting staff and performance concerns to the LSW or RM
Maintaining excellent communication with the entire LCC team on the daily basis to ensure good effective practice
Measuring effectiveness of plan by evaluating the results of support with RM; providing support improvement recommendations
Act in accordance with LCC policies and procedures also in line with all legal requirements; standards of support that comply to the values of LCC
To provide personal support and support SU with a wide range of needs, illnesses and disabilities, to know and understand the support and support of the SU and undertake the tasks detailed in the SU’s support and support plan using a person-centred approach and in the least intrusive way
To encourage the independence and motivation of the SU and not foster dependent behaviour
To provide input into the care and support plans of SU by regularly feeding back to LSW or RM
To give personal support / care to SU
To assist SU with their meds at the agreed level of support
To assist SU where required in the preparation of food and drink, being aware of the SU’s choice, likes/dislikes, nutritional needs and cultural requirements
To support SU with light general household domestic duties, including housework and laundry, as detailed in the support plan or instructed by Management
To use manual handling equipment safely and correctly
To take responsibility for the safe handling of property and equipment belonging to the SU
To maintain good communication and develop effective working relationships with SU, provide companionship, actively talking and listening to them about their interest and maintain contact with their family and friends
Meeting SU and their families to inform them of the role of a support worker; commencing the support plan following senior management assessment; offering information regarding treatments, procedures, prescriptions, and continuing Support
Accompany the SU on trips into the community
To ensure as safe as possible the living environment for the SU, whilst respecting the SU’s choice and rights
Recording and Reporting
To maintain detailed and accurate records on the Support Management System in respect of support in a timely manner and medication support given and tasks undertaken accordingly
To ensure the safe use of SU personal information according to current GDPR policies
To regularly read support and support plans, acknowledging changes
To protect the confidentiality of all information relating to the SU and not divulge information to anyone who is not authorised to receive it
To promptly report to the Senior Management team any issues concerning the support, well being or behaviour of the SU and update records accordingly and monitor where concerns have been reported and recorded
To recognise the signs of abuse and immediately report abuse/ suspected abuse /complaints to LSW and RM

Previous recent care experience at least 2yrs,
Health and Social Care qualification (minimum level 2 e.g NVQ2),
Medication Administration,
Have excellent written and verbal communication skills, Authorised references (2)

JOB HOURS – The role is to work as part of a service where you will be required to complete shifts based on your rota including weekends. *Shift patterns are dependent on operational capacity therefore they could change

7am – 3pm*
Shift 2
2.30pm – 10pm*
Shift 3
9.30pm – 7.30am*
