About this role
To work alongside and in support of the current GP with Extended Role Clinical Lead of the Chronic Fatigue Syndrome (CFS) Diagnosis Service in providing the diagnostic element of the CFS pathway Hertfordshire wide, including triage of referrals, Advice and Guidance, and face to face diagnostic assessments.
Provide medical input to the ACTIVATE for CFS and Persistent Pain multidisciplinary team within Hertfordshire Community NHS Trusts integrated Musculoskeletal Service serving the population of East and North Hertfordshire.
The post holder will work within a multidisciplinary team to deliver appropriate care to the patients referred into the service and provide medical expertise in diagnosis and MDT medical support.
1. Undertake comprehensive medical assessment of patients with suspected CFS, to include a thorough review of the patient’s ideas, concerns and expectations; history; systems review; symptoms; investigations; medications; examination findings. Formulate a full clinical plan including any differential diagnoses or suggested onward investigations/referrals. Refer on to therapy services if appropriate.
2. Adopt a holistic and compassionate approach to complex patients who may have had difficult prior experiences and present with medical complexity.
3. The post holder will enter full details of history, examination findings, diagnosis and treatment plan in the clinical record on SystmOne (Patient Recording System).
4. Ensure that patient or carers are informed fully about the condition, answer any questions, supply written information if available and involve them in decisions about the treatment.
5. Create a letter to the patients GP at the time of the consultation for every patient in accordance with service procedure.
6. Triage referrals received, reviewing pre-referral investigation results, and following service criteria
7. Respond to Advice and Guidance requests on ERS within their competence
8. Respond to medical queries and requests for medical reviews received from members of the Therapy services
9. Work with other members of the team to deliver a high quality, patient centered service which will include attendance at Operational meetings, Educational meetings & MDT meetings.

1. Participate in data collection including patient surveys and HCT audits as requested.
2. Comply with GMC standards of Good Medical Practice and the NHS constitution.
3. Undertake all required GPwER training and development to maintain skills/accreditation.
4. Maintain up to date knowledge of current NICE guidelines and keep themselves abreast of any new developments in the management of the conditions.
5. Attend regular doctors’ meeting/peer supervision for case discussion and mutual support with Clinical Lead GPwER
