Social Prescriber - Unity Healthcare

Social Prescriber We are looking for a Social Prescriber to join our team based at Unity Healthcare via our PCN covering Glemsford, Long Melford, Lavenham and some time in Haverhill.

Contract type

We are looking for 1 person to join us on a permanent part time basis as follows: 30 hours for either four or five days per week. We are happy to discuss how these hours are worked and we are flexible.
