An exciting opportunity has arisen for a dynamic, motivated and innovative Senior Pharmacist to join the expanding Medicines Management team at Central London Community Healthcare NHS Trust.

This role would suit pharmacists who are seeking a rewarding career in community healthcare and are keen to apply their expertise in a varied and patient focused environment. Candidates with experience in secondary care who are looking to transfer to a patient-centred environment and further develop their skills including service transformation, mobilisation/demobilisation projects, quality improvement, aswell as further developing leadership and management skills, are warmly encouraged to apply.

You will join an established, supportive and experienced team to enhance the service provided, with opportunities to use your existing skills and knowledge, while continuing to develop professionally within a growing service.

  • Support and deliver a high quality clinical pharmacy and medicines optimisation service to patients in Community Hospitals.
  • Ensure an integrated approach to medicines management and continuity of pharmaceutical care for patients admitted from primary and secondary care into Community Hospitals.
  • Contribute to the Trust’s clinical governance through interventions that optimise medicines management across all relevant areas of practice.
  • Lead, manage and develop staff within the Bedded Services team who work in Community Hospitals

The post holder will work as an active member of the Medicines Management team, contributing to the delivery of a safe, high quality and legally compliant service that aligns with national and local policies and priorities.

Depending on development and service needs, the post holder will be expected to work across Central London Community Healthcare Trust locations within the London area and may at times be required to work at other CLCH sites/localities to ensure Community Hospital patients have access to high quality clinical pharmacy services. Flexibility will be required as the organisation evolves.
