About this role

We are seeking a motivated, dynamic, and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team, the team deliver responsive high quality nursing to patients livingin Sandy, Potton, Gamlingay and surrounding villages within Mid Beds.We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs.

This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams

Youmusthave a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.

This is a full-time 37.5 hours per week permanent post.
