Practitioner to join our dynamic team. This pivotal role involves providing
essential direction, guidance, and support to our care teams, ensuring
seamless, high-quality care interventions for our residents.
As an Assistant Practitioner, you will play a crucial role
in enhancing our residents' well-being through your advanced training and
clinical expertise. You'll work closely with Registered Nurses and Unit
Managers, contributing to both team development and direct resident care.

Key Responsibilities Include
- Direct
Care and Support
Delivering best practice care, assisting
residents with daily living, reviewing care plans, and supporting social
activities.
- Clinical
Interventions
Performing trained clinical tasks such as
medication administration (including injections, insulin), oxygen
administration, catheter care, PEG management, wound care, and more.
- Team
Leadership
Providing clear direction, guidance, and support to
Health Care Assistants, fostering development, and educating care teams,
residents, and families.
- Quality
Assurance
Contributing to risk assessments, ensuring accurate
record-keeping and communication, and maintaining high standards of care.

Compliance
Adhering
to all Oakland Care policies, including safeguarding, health and safety,
and promoting safe working practices.
What We're Looking For
Essential Qualifications,
Knowledge and Experience
- Substantial
and demonstrable experience in delivering care.
- Level
3 in Health and Social Care.
- Successful
completion of the Oakland Care Assistant Practitioner training course or evidence
of an equivalent recognised qualification.
- Evidence
of competence and certification of all clinical skills required for this
role.
- Completion
and competent in medication administration either Guardian Angels
medication training or equivalent.
- Experience
in team member induction and supervision.

working knowledge of Dementia.
