All about the Role

The Community Long Term Team are looking for an experienced social worker to join the team. The Community Long Term Team supports all adults (18 and above) including those living with physical disabilities, older adults, people with sensory impairment or a learning disability and/or Autism. This role is a full-time position.

The Community Long Term Team work within the parameters of The Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, Human Rights Act 1998 and other relevant legislations. The role involves completing Care Act Assessments, Reviews, Mental Capacity Assessments, S117 Reviews, S42 Enquiries (and other relevant Safeguarding work), Copdol and other relevant and required court work. This is an exciting role which involves many opportunities for further growth and development of knowledge and skills.

All about our Team

We are a team made up of Social Workers, alternatively qualified roles, Senior Social Workers and three Team Managers. The Team Managers are Imaan Ashiq, Elaine Lounds and Zara Munt. The successful candidate will be within the line management of Imaan Ashiq.

As mentioned our team support a variety of social care duties and responsibilities in accordance with the Care Act 2014, and other relevant legislation. The Team support individuals to achieve their personal outcomes, work with unpaid carers as well as other statutory functions, such as adult safeguarding enquiries.

We are looking for someone with strong organisational skills and the ability to coordinate and manage the demands of a social worker role, with excellent communication skills and the ability to risk assess and analyse information to determine a direct course of action for those accessing our service. The service is busy, lively and interesting. The work is varied, offering the opportunity to become involved in court work, as well as other practice areas such as Continuing Healthcare and Adult Safeguarding work.

Your own progression is encouraged, with our current workforce having been supported to progress in a range of areas including AMHP training, Best Interests Assessor and Practice Educator qualification.

We are looking for a Social Worker who is committed to Social Work practice and is passionate about supporting others through their social care journey. We are looking for a Social Worker who is proactive and adaptable with a passion for ensuring that the adults receiving care and support in our area receive a positive experience when having their needs met. . The role is varied on a day-to-day basis and allows for a high level of multi-agency working, creativity and autonomy. The successful candidate will be part of an established team working across our Adult Social Care services. Applicants would need to hold a current Social Work qualification and registered with Social Work England. Experience of working with adults is essential along with a good understanding of Care Act 2014, Mental Capacity Act 2005 and other relevant legislation and be able to professionally utilise a Strengths Based Approach and be able to make risk assessed decisions to promote the safety of others. The successful candidate will need to be able to travel independently between locations including out of county areas on occasions.
