As a Community Palliative Care Specialist Nurse, you will be an integral part of ourintegrated Cancer, Non-Cancer, and Palliative Care team, working across organizational boundaries to deliver exceptional care.

This position offers a unique chance todevelop your skillsin palliative and end-of-life care, working alongside amulti-professional teamdedicated to providing the highest standard of care.

We are looking for a compassionate and dedicated nurse who is

Qualified with experience in palliative/end-of-life care

Organised and flexible, able to adapt to the needs of our patients

Compassionate towards patients and their families

Collaborative, with experience working in a multidisciplinary team

Proficient in IT, with experience using SystmOne

Excellent in communication, with strong community nursing skills

Skilled in triaging to ensure the best outcomes for patients

Holder of a full, valid UK driving licence and access to a car for business purposes (unless exempt under the Equality Act 2010)
