Physiotherapist - Willow Unit

Work as an active member of the Physiotherapy team, along with the other members of the Trusts Multidisciplinary team, organisations, carers. Assess for care packages for patients, for example with long termconditions, palliative care and rehabilitation needs, to achieve quality of life andindependence where possible. Work within the integrated team to facilitate early discharge from hospital. Work within the integrated team to prevent unnecessary admission to hospital
