Practice Nurse

The Practice Nurse will support the delivery of a range of clinical services including: Chronic disease management clinics, including Asthma, COPD, and Diabetes Health promotion and preventative healthcare Supporting patients with long-term conditions through education and care planning Delivering evidence-based nursing care within primary care guidelines Working collaboratively with the wider multidisciplinary team Maintaining accurate clinical records using NHS clinical systems Supporting quality improvement initiatives and patient safety standards Remote duties, where appropriate, may include chronic disease reviews, patient education, care planning, and virtual consultations.
