About this role
Key Duties and Areas of Responsibility In accordance with the practice rota, undertake duties including surgery consultations, triage, telephone consultations and queries. Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Screen patients for disease risk factors and early signs of illness. Assess the health care needs of patients with undifferentiated and undiagnosed problems. Visiting patients at home, checking and signing repeat prescriptions as well as dealing with paperwork and administration in a timely fashion. Perform clinical examinations of patients to assess, diagnose and monitor conditions. Carry out minor surgery where required. Provide emergency care while waiting for emergency transfer. Effective management of long-term conditions. Working collaboratively, accepting an equal share of the practice workload. Maintain accurate clinical records in conjunction with good practice, policy, and guidance. Undertake telephone triage and duty doctor roles. Liaise with other healthcare professionals as part of multidisciplinary teams. Develop care plans for health in consultation with patients and in line with current practice disease management protocols. Record clear and contemporaneous consultation notes to agreed standards. Prescribe in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate. Participate in practice audits as and when required. Participate in local initiatives to enhance service delivery and patient care. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA). Adhere to best practice recommended through clinical guidelines and the audit process. Contribute to the successful implementation of continuous improvement and quality initiatives within the practice. Contribute effective to the development and maintenance of the practice including clinical governance, training, financial management and HR. Prepare and complete the revalidation process. Health & Safety The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, to include: Using personal security systems within the workplace according to practice guidelines. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks. Making effective use of training to update knowledge and skills. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards. Reporting potential risks identified. Personal/Professional Development The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include: Completing all mandatory training on eLearning/Bluestream, ensuring that it is always up to date. Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
