The post holder will work with the Theale Medical Centre practice team to meet the needs of patients, delivery of high-quality patient focussed care to support families to stay well. Main Responsibilities We are looking for a committed, motivated Practice Diabetic Nurse to join our experienced and friendly established expanding nursing team. You will have a caring nature and a proactive professional manner to provide a high standard of nursing care. Duties will include management of Diabetic patients including monitoring, medications reviews and overseeing the recall process and practice nurse duties. You are further supported by an experienced admin and reception team. The role will include management of Diabetic patients including monitoring, medications reviews, overseeing the recall process and dietary and lifestyle advice. We have a phlebotomist who supports the Nurse Team. You must keep accurate records, attend practice and team meetings, as appropriate and carry out additional clinical activities or training if needed. Adhere to Practice guidelines, policies and procedures and infection control standards. As Diabetic Nurse, you will oversee the management of the Diabetic patients from recall, review and management. You will be supported by our HCA and GPs. Other responsibilities include keeping accurate records, working within agreed guidelines, appropriate administrative tasks, attendance and participation in practice and team meetings, as appropriate and carrying out additional clinical activities Adhere to Practice guidelines, policies and procedures and infection control standards. Ensure that organisational goals are reflected in own and the nursing teams objectives. Continue to develop professional and attend training courses and meetings. Maintain own professional and personal development in accordance with the NMC Code standards and professional guidelines. This role will also require you to work as part of the whole practice team and will include covering work for colleague absence. We are flexible on the hours and days that you wish to work.
