Assertive Outreach Support Worker

The post holder will work as part of the Community Mental Health Hub (CMHH), supporting the delivery of high-quality, person-centred care to adults with severe and complex mental health needs who may be at risk of disengagement from services.

Key responsibilities include

Supporting the delivery of recovery-focused, trauma-informed care in line with agreed care plans, under the supervision of qualified staff Providing practical, structured and time-limited interventions to promote engagement, stability and independence Working proactively within community settings, including home visits, outreach and community-based interventions, to engage service users Supporting individuals to develop and maintain daily routines, life skills and participation in meaningful activity Assisting with care planning, risk assessment and risk management, including identifying early signs of deterioration Promoting access to services, including health, social care and community resources, and supporting attendance at appointments Building therapeutic relationships to support engagement and positive behavioural change Working collaboratively within the multidisciplinary team and with partner agencies, including housing and voluntary sector services Acting as a link worker with identified local hostels to support engagement and continuity of care Maintaining accurate and timely clinical records in line with Trust policy Contributing to service development, quality improvement and team working
